Fully funded by the Ministry of Health and Long-term Care. Mississauga Halton CCAC provides clinical assessments and manages our diverse client needs to help them live safely at home or transition to long-term care facilities. We serve more than one million residents in South Etobicoke, Mississauga, Oakville, Georgetown, Halton Hills and Milton All CCACs are funded by Ontario's Ministry of Health and Long-Term Care at no direct cost to clients. We are best suited to provide care to high-need clients so they can remain at home as long as possible. We are not the most appropriate option for all seniors or citizens. When you contact us, you will be assessed and either assigned to a case manager or given information and referred to alternative community services. What we do, help people of all ages access the health and community care they need at home, at school and in the community. Assess clients, clinical and support needs.
